Early anticoagulation noninferior to delayed after ischemic stroke in patients with afib
No difference in a composite outcome of recurrent ischemic stroke, symptomatic intracranial hemorrhage, unclassifiable stroke, or systemic embolism was seen between patients with atrial fibrillation who received anticoagulation four or fewer days after an ischemic stroke and those who received it between seven and 14 days after, a British trial found.

Early anticoagulation appears safe after acute ischemic stroke in patients with afib
The AREST trial, which ended early due to changes in national guidelines, found statistically similar but numerically lower rates of the primary study outcome (fatal stroke, recurrent ischemic stroke, or transient ischemic attack) in those who received anticoagulation early with apixaban versus later with warfarin.

U.S. study finds gaps in access to mechanical thrombectomy for ischemic stroke
An analysis spanning 11 U.S. states found that 16% of patients with ischemic stroke from 2016 through 2018 were treated by EDs and hospitals that neither performed mechanical thrombectomy nor transferred patients to receive it.

Mobile stroke units led to better outcomes in patients eligible for tPA
A prospective, observational, multicenter study found that median time from stroke onset to administration of tissue plasminogen activator (tPA) was 72 minutes in patients treated by mobile stroke units and 108 minutes for those receiving standard emergency medical services.

Clinical decision tool may predict benefit of intra-arterial treatment after acute ischemic stroke
